Calculate does not save output base used with each history result

Description
Calculate does not save the output base desired with each entry item in the result history for the output, but does for the input.
This results in history entries which say "10 = 10" {when trying to convert decimal to hex} but also "0b011 = 3" {converting binary to decimal, or any output base for that matter}.
Seen with Calculate-40 in OLPC 13.1.0 os1.
